依頼内容のニュアンスについて

特に「計画 / 依頼 (Plan / Request)」の表現は、紹介先にどこまでお願いしたいのかを明確に伝える上で重要です。

assessment and advice on management: 評価と治療方針に関する「助言」を求める場合。その後の主治医は紹介元が担うニュアンス。
assume care for this condition / full assessment, investigation, management, and follow up: 診断から治療、その後のフォローアップまで、包括的に「今後の診療」をお願いしたい場合。

紹介の目的（一度きりの相談か、継続的な診療依頼か）に応じて、適切な表現を選びましょう。
